Heather Locklear has entered rehab to seek treatment for "anxiety and depression". The 'Melrose Place' actress has checked herself into a facility in Arizona to be cared for by professionals who will evaluate her medical condition.

Her representative Cece Yorke said: "Heather has been dealing with anxiety and depression. She requested an in-depth evaluation of her medication and entered into a medical facility for proper diagnosis and treatment. "This is a confidential medical matter and no further statement will be released."

The announcement comes just three months after a friend of the 46-year-old actress rang the emergency services, fearing Heather was suicidal and suggesting she was planning to overdose on her depression medication.

Following the incident, Heather revealed she was fine and did not need medical help for her condition. Heather - who recently finished filming US TV movie 'Flirting with 40' - has a 10-year-old daughter Ava with her ex-husband Richie Sambora. She is currently dating her former 'Melrose Place' co-star Jack Wagner.
