Entries can affect the play in a suit

Good bridge players have the funny habit of watching entries. Sometimes, though, the number of entries that you have in, say, the dummy, may affect not only your line of play but also the right way to handle a suit combination.
In today's deal, South is in three no-trump, and West leads the club queen. How should declarer plan the play? In particular, how should he handle the heart suit? And would his method vary if he had a second dummy entry?
